You are welcome to call CAPS if you have any questions with forms or to schedule a counseling appointment 407-823-2811. CAPS has both In-person or Telemental health (video) services, but you MUST be in the State of Florida to receive counseling services. Call CAPS if you need to speak to a counselor today or if you want to schedule an appointment.

 

How do I make an appointment for counseling?

 

For First Time/New Clients wanting to meet with a counselor for a one time, solution-focused Single Session (30-45 minutes).

 

Focus can include talking about your academic distress, concerns about how to help a family member or friend, anxiety or stress management strategies, and obtaining a referral for a counselor who can provide a letter for your emotional support animal, a referral to a specialist or a long-term community mental health counselor, a referral to a medical professional (psychiatry), or a referral for testing/assessment for ADHD, autism, learning disorders, etc. Please call CAPS at 407-823-2811 to schedule your Single Session appointment. Please inform the support staff that you are interested in this type of appointment.

 

For First Time/New Clients wanting Brief, Short-Term Therapy

(meets approximately every two weeks depending on treatment plan, and within a total of 6 sessions):

During the last month of the semester, students interested in seeking counseling services should call CAPS at 407-823-2811 or visit one of our offices in person. 

For Former Clients:

We are excited to welcome you back to CAPS! Please note, starting Fall semester 2023, if you are returning back to CAPS and you have existing CAPS charges on your account, you will need to address these charges prior to being able to schedule an individual, relationship, or group counseling session. If you are unsure whether you have any CAPS charges, please call us at 407-823-2811.

If you have zero charges on your CAPS account, then there are three different ways you can re-establish counseling services at CAPS. You are invited to:

  1. Visit us in-person
  2. Call us at 407-823-2811, or
  3. Utilize our online request form. First obtain the password to enter our online system by clicking HERE then fill out the form by clicking Former Client Form.

 

What happens after I made an appointment?

On the morning of your appointment. CAPS will send you a text with instructions or what form(s) are required to be completed. Please check your text messages.

To submit one of the forms below electronically, please 1) download the form, 2) open in Adobe Reader (not in the web browser), 3) complete all the items, and 4) sign the form digitally. Then 5) Save the document to your device, and 6) upload the completed document into the Qualtrics link found at the bottom of the page.

Only drop your forms during CAPS business hours (Monday-Thursday 8:00 am to 5:15 pm and Friday 8:00 am to 4:30 pm).  There will not be a therapist available to review documents outside normal work hours.  If you need immediate assistance call CAPS at 407-823-2811 during regular business hours, press 5 after hours, or if this is a life threatening emergency call 911 or go to your local hospital.  Please note that Qualtrics is not a secure form of transmitting confidential information.
