Triage Packet-Returning Client
Please submit your forms during CAPS business hours (Monday-Thursday 8:00 am to 5:15 pm and Friday 8:00 am to 4:30 pm). There will not be a therapist available to review documents outside normal work hours. If you need immediate assistance call CAPS at 407-823-2811 during regular business hours, press 5 after hours, or if this is a life threatening emergency call 911 or go to your local hospital. Please note that Qualtrics is not a secure form of transmitting confidential information.
