You are welcome to call CAPS if you have any question with forms or to schedule a counseling appointment 407-823-2811. CAPS has both In-person or Telemental health services, but you MUST be in the State of Florida to receive counseling services. Call CAPS if you need to speak to a counselor today.
How do I make an appointment for counseling?
For First Time/ New Clients:
You are invited to schedule your intake using our online scheduling system. You will be asked to complete the informed consent forms, then you will select a date, time and modality (in-person or video) for your intake appointment. Please notate your appointment information in your calendar/phone.
If you are a minor (under 18 years of age), or a Valencia Downtown student, or a couple wanting couples therapy, then you will need to call us directly to schedule your intake appointment. Call 407-823-2811.
What happens next?
You will receive a text the morning of your intake appointment with instructions on how to complete your online intake forms. You must complete your forms by 10 am the morning of the appointment, or you may lose your appointment time and you may be charged a fee. We are a highly utilized counseling center, and want to make our services available to as many students as possible. This requires your assistance in completing your forms prior to your appointment so that our counselors are ready for your counseling session. In selecting an intake appointment, please choose a day that allows you sufficient time to complete all our forms; on average, you will need at least 30 minutes to complete the forms in the morning.

For Former Clients:
There are three different ways you can re-establish counseling services at CAPS. You are invited to:
1) visit us in-person,
2) call us at 407-823-2811, or
Once CAPS receives your request to re-establish services, we will contact you via text or phone to schedule an appointment.
What happens after I made an appointment?
On the morning of your appointment, CAPS will send you a text with instructions on what form(s) are required to be completed. Please check your text message.

FREQUENTLY USED FORMS:
To submit one of the forms below electronically, please 1) download the form, 2) open in Adobe Reader (not in the web browser), 3) complete all the items, and 4) sign the form digitally. Then 5) Save the document to your device, and 6) upload the completed document into the Qualtrics link found at the bottom of the page.

Only drop your forms during CAPS business hours (Monday-Thursday 8:00 am to 5:15 pm and Friday 8:00 am to 4:30 pm). There will not be a therapist available to review documents outside normal work hours. If you need immediate assistance call CAPS at 407-823-2811 during regular business hours, press 5 after hours, or if this is a life threatening emergency call 911 or go to your local hospital.
